There are two different formats; Online and Hard Copy

Online is an easy 2-step process; buy a Password, receive a Link and Password and give/send it to the Couple.  

Advantages:

Encrypted

20% -35% less expensive than the Hard Copy version

Couples enter ALL information-no clerical errors on your part

Once completed the Report is immediately available

Cell Phone Friendly

Hard Copy consists of Questionnaires and Answercards or complete Kits which include all materials necessary for one Couple. 

These need to be completed and mailed to our office.  The turnaround time is a week to ten days.

Disadvantages:
